A report will be submitted to the government and appropriate action will be taken at the state- level, Yavatmal Collector M D Singh said.
The incident took place at Bhanbora PHC in Kapsikopri village when the National Pulse Polio vaccination drive for children aged 1-5 years was underway.
Chief Minister YS Jagan Mohan Reddy launched the campaign by administering polio drops to children at his camp office on Sunday.
Vaccination teams have also been deployed at bus terminals, railway stations, airports and ferry crossing to vaccinate children in transit to ensure no child misses the life-saving dose.
The polio vaccine programme, which will cover 64,07,930 children in the state, will be flagged off by Chief Minister B S Yediyurappa at his home office.
Union Minister for Health and Family Welfare Harsh Vardhan had said on January 8 that polio immunisation will be carried out on January 17.
Other health workers raised questions about whether large immunisation camps could be held as it was until up to the present.
