Despite increasing number of coronary interventions over recent years, there are still a considerable number of patients suffering from chronic refractory angina pectoris.  Refractory angina pectoris, is defined as chronic chest pain or angina  resistant to maximal medical therapy and standard coronary revascularization procedures. It remains a significant health problem all over the world . For this challenging subset of patients, surgical and interventional options have been exhausted and therapy remains limited to the use of multiple anti-anginal drugs, decreased physical activity, stress level and life style modification.  

Patients with refractory angina pectoris in end-stage coronary artery disease represent a severe condition with a higher reduction of life-expectancy and quality of life as compared to patients with stable coronary artery disease. Refractory angina pectoris during end-stage CAD and with endothelial dysfunction is a specific coronary syndrome that is chiefly caused by microcirculatory disturbances. The majority of patients with end-stage coronary artery disease can be treated effectively with conventional non-invasive treatment modalities.

In patients with intractable angina refractory to aggressive surgical and medical treatment, several novel alternative strategies are considered which includes- EECP, percutaneous myocardial laser revascularisation, spinal cord stimulation, intermittent urokinase–therapy, sympathectomy and angiogenesis by gene or cell therapy.

A new Extracorporeal cardiac shock wave therapy (CSWT) also known as ESMR, has been introduced recently to improve myocardial ischemia in a patient with severe CAD and refractory angina pectoris. This new alternative treatment modality is gaining world wide acceptance for the management of Refractory angina with end stage coronary artery

disease.

Reversible ischemic regions no longer treatable or approachable by invasive methods are being targeted and treated with Extracorporeal shock wave therapy.  Extracorporeal Shock-Wave Myocardial Revascularization (ESMR) is an effective and patient friendly modality for the stimulation of angiogenesis and for treatment of patients with refractory angina and is poised to become a more widely used treatment for the growing population of patients with refractory angina.   ESMR treatment is being introduced and offered at NEOMED HOSPITAL, Annanagar, Chennai.
